Trying to get a Giant African. ppq 526 is confusing

Im trying to get my hands on a giant african pede and I was pointed in the direction of Wards:

I contacted them and they said I need to get a permit from USDA by filling out a PPQ 526. The form is quite confusing. Has anyone gone through this proccess or does anyone have an words of encourgament?

My Girlfriend and I go every time it comes around. Millipedes are few and far between unfortunantly. I saw one guy with them a few years back and he wanted $100 each!
